How to Create and Share a Home Exercise Program (Physitrack)

Physitrack allows practitioners to create personalized home exercise programs (HEPs) and share them with patients. When integrated with Juvonno, exercise programs can be created from a patient's profile, automatically saved to the patient's documents, and made available through the Patient Portal. This article explains how to create a home exercise program, access the completed program in Juvonno, and share it with a patient.

 

When to Use This

Use this article when:

  • You need to create a new home exercise program for a patient
  • You want to access a previously created Physitrack exercise program from a patient's profile
  • You need to share a home exercise program through the Patient Portal
     

What You Need

Before you begin:

  • The Physitrack integration must be configured 
  • Your practitioner profile must be linked to your Physitrack user account
  • The patient must have a patient profile in Juvonno
  • The patient must have access to the Patient Portal (only if sharing via the portal)  

 

Creating a New Exercise Program 

  1. Open the patient's profile 
  2. Click the down arrow on the Save & Exit button 
  3. Select Open in Physitrack 

 

 

  1. Physitrack will open in a separate window

  1. Create the exercise program in Physitrack

  1. Save the program in Physitrack
  2. Return to Juvonno and refresh the patient's profile
  3. Click the Documents tab to view the newly created program 

 

Sharing the Exercise Program in the Patient Portal 

  1. Open the patient's profile
  2. Click the Documents tab 
  3. Locate the exercise program PDF that was automatically added to the patient's documents
  4. Hover in the area below Visible in Portal 
  5. Click the + icon when it appears

 

 

  1. Confirm that a green checkmark appears

 

 

The green checkmark indicates that the document is available to the patient through the Patient Portal.

 

Result

The exercise program has been created in Physitrack, saved to the patient's documents in Juvonno, and shared through the Patient Portal. Patients can log in to the portal to view and access their home exercise program.

 

Avoid Common Issues

  • Verify that your practitioner profile is linked with Physitrack 
  • Refresh the patient's profile after saving a program in Physitrack if the program does not immediately appear
  • Confirm the document has a green checkmark under Visible in Portal before advising the patient to access it online

     

Troubleshooting

I created a program in Physitrack, but I do not see it in Juvonno

  • Refresh the patient's profile and reopen the Documents tab. If the program still does not appear, verify that the program was successfully saved in Physitrack.

The Physitrack window does not open

  • Make sure pop-ups are not being blocked by your browser
  • Confirm that the Physitrack integration has been configured and that your practitioner profile is linked with your Physitrack user account 

The patient cannot see the exercise program in the Patient Portal

  • Verify that the program document has been shared by confirming a green checkmark appears under Visible in Portal
