How to Generate a Letter with Chart Notes

The Chart Note Go Code allows you to include information from a patient's chart directly in a letter. This is especially useful when creating referral letters or other clinical correspondence because it eliminates the need to copy and paste chart notes into the letter manually. When the letter is generated, the system inserts the contents of the selected chart's Notes section exactly as they appear in the patient's record.

 

When to Use This

Use this article when you want to:

  • Include chart notes in a Letter (e.g., referral) 
  • Reduce duplicate documentation
  • Reuse clinical notes that have already been entered into a patient's chart
  • Create letters that summarize a patient's visit
     

Permissions

To complete these steps, you need:

  • Access to Forms & Letters if creating a new template 
  • Permission to generate letters from patient charts

Contact your clinic administrator if you do not have the required access. 
 

Adding the Chart Note Go Code to a Letter Template

  1. Go to Settings
  2. Under Forms & Letters, select Letters

 

 

  1. Create a new text letter template or open an existing one
  2. Place your cursor where you want the patient's chart notes to appear

  3. Insert the following Go Code: {goChartNote}

     

 

  1. The Go Code will populate in the letter 

 

 

  1. Add any additional text or Go Codes that you want included in the letter
  2. Click Save

 

Generating a Letter with Chart Notes

  1. Open the patient's profile
  2. Go to Documents 
  3. Click +New Letter 

 

 

  1. Create a letter using the template containing the {goChartNote} Go Code

     

 

  1. Select the Related Chart that contains the notes you want to include in the letter 

 

 

  1. Complete the remaining details
  2. Generate the letter

 

 

Using the Chart Note Go Code with AI SOAP Generation

If your clinic uses AI SOAP generation, the Chart Note Go Code can help streamline referral workflows even further.

After completing a patient's chart, the practitioner can generate an AI SOAP note and save it to the chart's Notes section. When a letter is created using a template containing {goChartNote}, the generated SOAP note is automatically included in the letter without needing to be copied and pasted.

This can be especially useful for referral letters, where the receiving provider needs a summary of the patient's visit.

 

Result

Your letter includes the contents of the selected chart's Notes section and appears in the Documents tab in the patient's profile.

Avoid Common Issues

  • The Chart Note Go Code only pulls information from the Notes section of the selected chart
  • Select the correct chart when generating the letter to ensure the appropriate notes are included
  • Any formatting or text in the Notes section is carried into the letter as it appears in the chart
  • Review the generated letter before sending it to ensure the included notes are appropriate for the recipient

 

Troubleshooting

Why aren't my chart notes appearing in the letter?

  • Confirm that your template contains the {goChartNote} Go Code
  • Verify that you selected the correct chart when generating the letter
  • Ensure the selected chart contains information in its Notes section

Why isn't information from other chart sections included?

  • The Chart Note Go Code only retrieves content from the Notes section of the selected chart

My AI SOAP note didn't appear in the letter

  • Ensure the AI-generated SOAP note has been saved to the chart's Notes section before generating the letter
  • Verify that you selected the chart containing the completed note
