- When forms are accidentally filled out under another family member’s account (e.g., parent, spouse)
What You Need
The destination patient profile must already be created
Permissions
- Your User Type must have access to Reassign Intake
Reassigning an Intake Form
Follow these steps to reassign an intake form:
- Open the patient profile where the intake form is currently located
- Click Intake
- Click the options icon next to the intake form
Select Reassign Intake
- Begin typing the name of the patient you want to assign the form to
- Select the correct patient from the list
- Click Reassign Intake
- Review the selected patient to ensure it is correct
- Click OK to confirm
Result
- The intake form is moved to the selected patient’s profile
It is removed from the original patient’s Intake tab
