How to administer custom forms to the patient profile and on the portal.
This section will show you how to enable forms in the patients profile and in the portal individually per Patient.
Step 1
Open the Patient profile and go to the Intake Tab.
Step 2
On the far right side of the page select "Administer forms".
Step 3
You will get a popup with a list of forms to show in the patient profile and the portal. There is a default set based on what was selected under Settings>Intake Forms & Chart Templates>Intake Defaults link.
You are able to edit what is hidden and showing by selecting it from the drop down arrows. When finished selected Save.
