Enable Claim and Consent Forms

Juvonno has over 100 Claim and Consent forms from various providers all of which are previewable with a single click. This article will walk you through How to Enable Claim and Consent Forms

Instructions

1. Go to Settings 

2. Under Forms & Letters, select Forms 

    

3. Click on the name of the form to preview any of the available forms 

    

💡Tip: Use the Search Bar or select a Category to find forms quicker

4.  Click the Checkbox beside each form you'd like available in your system

5. Click Save

    

Result

Enabled forms will appear: 

• Inside the Patient Profile under Forms

• During the Checkout process for Claim Forms

• Some forms are linked to the Billers you have enabled in Juvonno including: 
  • HCAI 
  • OHIP
  • WSIB
  • WCB 
  • Alberta Health
