How to Set Up Intake Form Defaults

You can set up which intake forms appear in the Patient Profile, the Patient Portal, and which are automatically selected when sending a Welcome Packet to a patient. These intake form defaults can be customized to send specific forms based on the service a patient books, ensuring they only receive the most relevant forms.

 

Permissions 

  • Access to Forms & Letters (User Type setting)

     

Setting Up Intake Form Defaults 

  1. Click Settings 
  2. Under Forms & Letters, select Intake Form & Chart Templates.
  3. Click Intake Form Defaults 

     

     

  4. Choose which intake forms you would like to appear in the Patient Profile and in the Patient Portal
    • Show in Profile - Select this if you want this form to appear in the Patient Profile
    • Show in Portal - Select this if you want this form to appear in the Patient Portal  
      • Intake forms selected to Show in Portal will automatically be selected when sending the Welcome Packet so we recommend only selecting intake forms that are relevant for everyone

In the following example, all of the intake forms will appear in the Patient Profile and the Health History would be pre-selected to be sent in each Welcome Packet. 

You would then select any other relevant forms to include in the Welcome Packet at the time it is sent. 

 

 

  1. Save your changes or 
  2. Continue to customize:
    • Default Clinic
      • Multiple clinics: If intake form is relevant to only one clinic, you can select the clinic or leave this as All
      • Intake form will only appear in the patient profile of those patients with a Default Clinic that matches 
    • Default Product Category
      • Select the Product Category that the form relates to 
      • When a patient is booked in for a service that's included in the Default Product Category, the Welcome Packet will pre-select the relevant forms 

         

In the following example, when a patient books anything that falls under the Physiotherapy Services category, the Physiotherapy Consent will be pre-selected in the Welcome Packet sent from the appointment. 

 

 

  • Default Products Items - You can select Default Product Items if the form is relevant for particular services only 

In the following example, when a patient books a Pelvic Physiotherapy Assessment, the Pelvic Health Physiotherapy - Intake and Pelvic Health Physiotherapy - Consent will be pre-selected in the Welcome Packet sent from the appointment. 

 

 

 

Troubleshooting

All intake forms are appearing as pre-selected, even though defaults have been configured.

  • Make sure Show in Portal is deselected for any forms you do not want to be automatically included. By default, any forms set to display in the portal will be automatically pre-selected when sending a Welcome Packet.
