When a patient is booking an appointment through the patient portal they have the option to include a message that is directly put on the appointment schedule.

The Patient leaves a message on the Review your booking page inside of the patient portal.

Clinic Appointment on Schedule
After the appointment has been accepted (if the setting turned on) or directly placed on the schedule you will be able to see the message that the patient has left in the Memo section of the Appointment Details.
