Steps
1. Go to the patient's profile.
2. Select the Intake tab.
3. Select the "Administer Forms" button located on the right side of the page.
4. Forms you have published will appear on the list. You can select to show or hide the form in the portal. If the form is 'visible' in the portal, the select patient will be able to fill out or read the intake form.
5. Save.
Drop-down Menu Options
1. Hover over the entry you want to edit and select “Edit”.
2. Edit the page then select “Save”.
